Access to Care and Continuity of Care

While providing care services, our hospitals aim for a more comprehensive and integrated approach. Our goal is to match the patient's health and care needs correctly with the available services, to coordinate the services offered to the patient within our institution, and then to make the necessary plans for the discharge and follow-up process.

Ensuring that the services and care patients need are provided without interruption from admission to the hospital until discharge; patient admission and hospitalization, discharge, referral and follow-up procedures, and transfer processes within or outside the institution are the committee's main areas of work. In this context, the committee carries out the following activities:

  • - Ensuring that triage criteria are determined and implemented in the admission of emergency patients, and that the patient is informed if there is a delay or disruption in the service to be provided,
  • - Ensuring that a standard patient admission process is implemented for outpatients and inpatients,
  • - Determining the process for the flow of patients between hospital units in accordance with legislation and standards,
  • - Ensuring that patient admission and discharge criteria are determined and implemented for intensive care units and specialized units,
  • - Ensuring that an effective coordination mechanism is established between hospital units,
  • - Ensuring that, when patients are transferred between units, patient information is handed over together with the patient, and that the patient transfer process is defined and implemented,
  • - Ensuring that the patient's discharge is planned in the early stages of hospitalization,
  • - Ensuring that, when a patient is referred, a planned referral procedure is carried out in cooperation with the receiving institution,
  • - Ensuring that, for all inpatients, at the end of the hospitalization process an epicrisis (discharge summary) containing summary information on the services received during the hospitalization is prepared and added to the file,
  • - Ensuring that a clinical summary is prepared for patients receiving complex and long-term outpatient care,
  • - Ensuring that the process is defined and implemented for patients who wish to leave the hospital against medical advice,
  • - Ensuring that welcoming, information and guidance services are provided in the hospital to make it easier for patients to access services,
  • - Ensuring that the necessary arrangements are in place so that patient registration procedures can be carried out effectively and accurately,
  • - Ensuring that arrangements are in place for appointment and result-reporting processes for diagnostic and treatment purposes,
  • - Ensuring that arrangements are in place to make it easier for elderly and disabled people to access health services.
